By signing this Fundraising Program Agreement, I _______________________ (“Fundraising Representative”) am confirming and verifying that I am an authorized representative of ____________________ (Organization Name), the re- sponsible party (“Responsible Party”), and that I have the right and authority to enter the Responsible Party into the fol- lowing binding agreement, including the establishment and maintenance of a Fundraising Campaign with Quick Quack Car Wash. 

 I also acknowledge that I have read, understand and agree to the terms and conditions set forth with regard to
establishing and maintaining a Quick Quack Car Wash Campaign.

1. In the event thatthe FundraisingOrganization isunable to sell the entire quantity of passes withintheallotted time, the Responsible Party agreesto the following: 

• All unsold Unlimited Membership Cards must be returned to Quick Quack Car Wash within 14 business days of fundraising completion and agreed upon date. 
• A check or money order totaling the total proceeds from sold passes must be submitted with the returned unsold passes. (For every pass sold, The Responsible Party gives $5 toQuick Quack, andThe Responsible Party keeps $5). 

2. The Responsible Party takes full responsibility for the retail value of any quantity of unreturned passes. There are no exceptions to responsibility for the passes(i.e. lost, stolen, damaged, etc.) At the discretion of Quick Quack Car Wash, passes may not be honored at any location or canceled should a problem arise with the Organization’s account. 

3. The Responsible Party understandsthat the Unlimited MembershipCards are not redeemable for cash; however, they can be used at their retail value to upgrade to a higher-level package, with the holder of the certificate then paying the difference. Unauthorized or reproduced wash passes will not be honored for redemption. 

4. The Responsible Party understands that when redeeming the card at any Quick Quack location, a $1 fee is required for activation. The Guest will be enrolling into a monthly recurring membership. After the first 30 days of their membership, the Guest's credit card will be charged Full Price/ the retail value of the membership. Please see a cashier for details or visit dontdrivedirty.com.. If they wish to cancel your recurring membership, they may visit any Quick Quack Car Wash location, or call our Customer Service line at(888)772-2792. If the Guest chooses not to pay the $1 activation fee, they may redeem the voucher for One Single Lucky Duck Wash, valid only one time at a single location. 

5. The Responsible Party understandsthe following: 

• Quick Quack CarWash Unlimited Membership Card may not be discounted below the current retail price and may not be sold on the premises of any Quick Quack Car Wash location. 
• Organizations are allowed to participate in four Fundraisers per calendar year with Quick Quack Car Wash. 
• Fundraiser Unlimited Cards are solely intended for individual consumers use only. 

6. The Organization agrees that the promotion and sale ofQuick Quack Car Wash passes will be monitored and controlled by the standards set by Quick Quack Car Wash. The Organization accepts all terms noted in this contract, accepts full financial responsibility for any unauthorized distribution or redemption of passes, and return of unsold passes and monies upon completion of the fundraising campaign. 

COST OF COLLECTION: In the eventit is deemed necessary by Quick Quack Car Wash to institute collection proceedings against the undersigned for any amounts owed, the undersigned agrees to additionally pay Quick Quack Car Wash for all costs and expenses of collection, including without limitation all reasonable attorney fees, expenses, court fees, and costs of collection. 

CERTIFICATION: I/We certify that all information on the application is correct and that an authorized officer, owner, partner orrepresentative has signed this application. I/WE CERTIFY THAT THIS APPLICATION IS FOR FUNDRAISING AND IS NOT FOR PERSONAL, FAMILY OR HOUSEHOLD PURPOSES. By signing below, I/we acknowledge and agree to the foregoing conditions.
